I, the Proctor, understand that each application must include: second page of NIDE Examination (the "Passing Score Acknowledgement"); a completed application (the "NIDE Registry Application"); and documentation of training (a “Certificate of Completion”). *

Prerequisites for Certificate: I, am not required to be licensed or certified to hold my current position, and if I am, I have the proper license and/or certification. I am obtaining a NIDE certificate as an important indicators of dementia competency and further my ability to work with dementia. The NIDE certificate does not license, certify, approve, or confer authorization to anyone the right to practice where such license or certification is regulated by state, municipality, or government entity. No Guaranties Or Warranties: I understand that NIDE and the NIDE SOE Council have made every effort to ensure that the information provided is accurately represented the Program. NIDE does not make any guarantee nor warranties of the following: the accuracy; reliability; completeness; currentness; or timeliness of the content; software, documents; articles; text; graphics; links; or communication provided on or through the use of the literature on the NIDE website, through any NIDE approved Programs, by the NIDE approved educator; or a collaborative organization.
